Forms & Function

A necessary function for school is to insure that all students have current immunization forms on Georgia Form 3231.

These forms are required to be on file with the school by the State of Georgia before your student can attend class.

Please make sure that we have your child’s form no later than August 12th. Additionally, a copy of a birth certificate is necessary for all new students.

Immunization Forms Needed!

At your child’s annual check-up, please make sure to get a copy of the Georgia immunization form 3231. Students are required to have a current immunization form on file prior to the beginning of school.
